We have included all the forms that you will need for your program. Client Info and Update Info are
electronic-online forms available to you after logging into myFHS.org. After you are enrolled in the
program you may add and delete creditors to your program with our electronic-online form Add Creditor
(uou will need your client ID).
Automated Clearing House (ACH) Form #ES3983 authorized Financial Help Services to make electronic
bank transfers. The Creditor Listing Form needs to be mailed to FHS for the processing of your creditor
information. Our Monthly Budget Worksheet maps your income to your expenses.
